WhatsApp Customer Feedback Form Template
Feedback forms fail when they are too long or land somewhere nobody checks. This one asks five short questions and delivers each response to your WhatsApp, so an unhappy customer reaches you while you can still do something about it.
Ideal para
- A QR code on a receipt, table tent or delivery bag
- A follow up message sent a day after a purchase or a visit
- A post visit survey for a clinic, salon or workshop
Lead with the star rating. It is the one question almost everyone answers, so putting it first means you still learn something even from the people who abandon the rest of the form. Keep the open comment box optional for the same reason.
The recommend question is a plain zero to ten scale, which is what an NPS score is calculated from. If you are not tracking NPS, delete it. A shorter form beats a metric nobody looks at.
