WhatsApp Appointment Booking Form Template

Booking by phone means missed calls, and booking by chat means twenty messages to pin down one slot. This template collects the service, the preferred date and time and the contact details in one pass, then sends the request to your WhatsApp so you only have to reply yes or offer another time.

Great for

• A salon, barber, spa or nail studio taking appointments
• A clinic, dentist or physiotherapist handling booking requests
• A photographer, tutor or coach booking one to one sessions

This is a request form, not a live calendar. That is the point: you keep control of the diary and confirm each slot yourself in the chat, which is how most small studios actually work. Set the date question to block past dates so nobody asks for last Tuesday.

Edit the service list to match your price list and keep it under about eight options. If you offer more than that, group them, and let the customer describe the detail in the notes question at the end.
